Determinação da sensibilidade a amoxicilina e a clindamicina do staphylococcus spp isolado da cavidade oral em pacientes com alto risco de endocardite infecciosa

Na literatura há poucos estudos sobre a distribuição do Staphylococcus spp na boca e os que existem sobre estes microrganismos como moradores da cavidade oral são controversais. O gênero Staphylococcus spp e parte de um importante grupo de patogenos no ser humano e provoca várias doenças, entre elas a endocardite infecciosa, também a literatura relata taxas de resistência aos antibióticos amoxicilina e clindamicina normalmente utilizados para a profilaxia em odontologia. Objetivo: conhecer a sensibilidade do Staphylococcus spp a amoxicilina e a clindamicina isolados da cavidade oral em pacientes com alto risco de endocardite infecciosa. Materiais e Métodos: Foram incluídos 46 pacientes maiores de idade que foram ao Serviço de Estomatologia e Cardiologia do HOCEN durante o mês de agosto de 2010, que preenchiam os cri térios de pacientes com alto risco de endocardite infecciosa. Foi uma raspagem da cavidade oral com um cotonete, e se cultivou em ágar sangue carneiro, foram identificados Staphylococcus spp. Foi realizado antibiograma para determinar a sensibilidade com o método de Kirby Bauer. Resultados: Dos 46 pacientes, em 28,2% foram identificados Staphylococcus spp. Destes 61,5% foram Staphylococcus aureus, 30,7% S. S. epidermidis e 7,6% hominis. O 30,8% dos Staphylococcus spp foram resistentes à clindamicina e 76,9% a amoxicilina. Conclusão: Este estudo mostrou que a amoxicilina e a clindamicina utilizados na profilaxia antibiótica em odontologia deixam uma grande porcentagem de microorganismos sem cobertura, como o caso do Staphylococcus spp, o que faz necessário avaliar o uso de uma profilaxia antibiótica com uma maior cobertura.
